Replace __CLASS_* and ____CLASS_* patterns with PROXYSQL_*_H to comply with the C++ standard where identifiers starting with __ are reserved in the global namespace. Also fixes: - ____CLASS_STANDARD_MYSQL_THREAD_H -> PROXYSQL_STANDARD_MYSQL_THREAD_H - ____CLASS_STANDARD_PGSQL_THREAD_H -> PROXYSQL_STANDARD_PGSQL_THREAD_H - __CLASS_PROXYSQL_POLL -> PROXYSQL_POLL_H (added _H for consistency)	3 months ago
